Santa Anna ISD

All medication sent to the school to be administered must be in the original bottle.

The medication must be accompanied by a note from the parent/guardian stating the following:

• the child's name
• what the medication is
• what the medication is used for
• how often is the medication to be taken/administered
• what is the dosage to be given
• No medication will be given unless these steps are taken.

Any mediation given for more than 2 weeks must have a form filled out and signed by both the parent and the doctor.

Students may not carry medications on their person!
